During your PHB journey, regular reviews that focus on outcomes you want to reach, rather than processes, provide an effective way of identifying what is working well and what what isn't. 

This monitoring & review process helps identify: 

• any changes in your needs or circumstances
• whether your agreed outcomes are still appropriate
• any changes in risks or safeguarding needs
• if your budget is adequate

These reviews should take place on regular basis with your ICB to ensure your PHB is working for you.
